Plan of Care
A registered nurse will visit clients in their homes to assess their condition, progress, and response to the services provided.
Personal Care Services
Our Personal Care Services (PCS) provide assistance with daily personal care needs—such as meal preparation, feeding, bathing, dressing, toileting, and more—within the comfort of your home. These services are directly tied to medical conditions and delivered by trained paraprofessionals, not medical staff.
Activities of Daily Living (ADLs)
Basic personal care tasks performed daily, including ambulation, bathing, bed mobility, dressing, eating, personal hygiene, toileting, and transfers. These activities support medical diagnoses and are usually provided by paraprofessionals.
Pediatric/Adolescent PCS
Children with injuries or disabilities often need extra help with everyday tasks. Our team at A Special Place Home Care Homecare ensures children remain comfortable and can focus on building independence and a fulfilling life.
Senior/Disabled PCS
We understand that aging and disabilities can make personal care tasks challenging. Our mission is to help seniors and disabled individuals maintain their dignity and independence by providing respectful, compassionate assistance.
Companion/Sitter Care
A companion or sitter provides non-medical support, supervision, and companionship to seniors. Services can be full-time, part-time, or 24/7 in various settings, but usually in the home. Tasks are limited to non-medical, non-physical support, and may include light housekeeping and errands depending on the arrangement.
Respite Care
Temporary care for an elderly, ill, or disabled loved one, allowing their primary caregiver to rest or handle other responsibilities.
Private Pay Home Care
Hourly packages of home care services for those who need help with daily tasks like meal preparation, housekeeping, and personal hygiene. A caregiver visits your home or your loved one’s home for these services.
CAP/C (Community Alternatives Program for Children)
This program allows medically fragile children to remain at home rather than in an institution. Services provided by our company include:
- Pediatric Nurse Aide services
- Personal Care services
- Respite care
CAP/DA (Community Alternatives Program for Disabled Adults)
This program helps elderly or disabled adults remain at home instead of in an institution. It supplements existing support systems. Services provided by our company include:
- Personal Care services
- Respite care
Additional In-Home Services
- Supervision and companionship
- Assistance with bathing, dressing, and incontinence care
- Homemaking, light housekeeping, and meal preparation
- Shopping and errands
- Help with correspondence and bills
- Medication reminders
